International Shipping Rule not being applied consistantly

Here is a question that a client of mine has about International Shipping Rules. They are having the same problem on both websites they have that run your script.

Here it is:

"I have a question on the international shipping rule. I have it set up to charge 25% of the order total when the bill to or ship to country is not equal to United States. Sometimes it charges and sometimes it doesn't. There have been a couple of times when the customer has left the country field blank, but most of the time these are filled in. There is an order from today if you want to look at an actual order. He filled both the bill to and ship to fields in with Sweden and was not charged any shipping fee.

This rule is set up the same way on the other website and we have the same problem on that site as well. Is there another way to set up this rule?"

Any suggestions would be great! If it wasn't working at all that would be one thing, but the fact that is doesn't work SOMETIMES is puzzling to me.
